ATTENDEES WILL LEARN PRACTICAL TOOLS FOR CONDUCTING A COMPREHENSIVE ASSESSMENT AND PROVIDING INDIVIDUAL AND GROUP PSYCHOTHERAPY WITH CLIENTS SEEKING TO IMPROVE THEIR SEXUAL HEALTH.

Out of control sexual behavior (OCSB) has been controversial for over 30 years. Pathology-based models (e.g., sex addiction, sexual compulsivity, or hypersexual disorder) are criticized for perpetuating socio-cultural sex negativity and inadequate scientific evidence to support establishing a new psychosexual or addictive disorder. In the absence of an accepted diagnosis or standardized treatment protocol, Mr. Vigorito discuss an integrative, clinical pathway to guide the assessment and treatment process with men concerned about sexual behavior problems and OCSB.

As opposed to a clinical disorder, OCSB is defined as a sexual health problem in which a man’s consensual sexual urges, thoughts or behaviors feel out of control (Braun-Harvey and Vigorito, 2016).  OCSB is conceptualized within a dual process model of human behavior (Loewenstein & O’Donoghue, 2007) in which “feeling out of control” is seen as the struggle to manage sexual motivations that compete with the person’s values and commitments. The OCSB Clinical Pathway was developed by Doug Braun-Harvey and Michael Vigorito over 20 years treating hundreds of men.  It involves a comprehensive screening procedure and assessment process based on Motivational Interviewing (Miller & Rollnick, 2013) and the transtheoretical model of health behavior change (Prochaska, Norcross & DiClemente, 2007). An OCSB assessment investigates how men resolve an array of competing motivations that affects self-regulation, attachment patterns, and their sexual/erotic identity development. The pathway is founded on the clinician’s ethical responsibility to provide the most effective treatment while protecting client sexual rights. Through combined group and individual psychotherapy, clients improve sexual control, clarify their personal visions of sexual health, and learn to align behavior within fundamental sexual health principles

ABOUT THE PRESENTER:

MICHAEL VIGORITO, MFT, CGP, CST-S

Michael Vigorito is a sex therapist, author, and trainer. In his practice, he provides individual, couples, and group therapy for a range of sexual and behavioral health concerns including problematic and out of control sexual behavior, sexual disorders, and sex-drug linked behaviors.

In addition to his Washington, DC private practice, Michael* developed a sexual health assessment and treatment program, which was published in Treating Out of Control Sexual Behavior - Rethinking Sex Addiction (SSTAR 2018 Health Professional Book Award Winner). 

Michael designed and implemented SAMHSA and CDC-funded training programs to integrate sexual health services into behavioral health care in the Baltimore and DC metro Areas. He is currently affiliated with University of Maryland - Prevention Research Center where he is implementing the Sexual and Gender Diversity Learning Community** – a CDC-funded, LGBTQ+ affirmative care training program for mental health providers and organizations.

Michael provides various sexual health trainings nationwide and was recently selected as a SSTAR Academy Speaker.

Wendy E. Miller PhD

Dr. Wendy Miller is a clinical psychologist with 35 years of experience treating 

individuals and couples.  She received her PhD  from George Washington University in 1985 and has a certificate in psychoanalysis from the NYU Postdoctoral Program in Psychoanalytic Psychotherapy and Psychoanalysis.  She has trained extensively in Emotionally Focused Couples therapy and sex therapy and is an AASECT Certified Sex Therapist.   She is on the faculty and a supervisor at The Women’s Therapy Centre Institute, a training center in NYC, where she teaches on sexuality and is on the Executive Committee of The Center for Optimal Living, a premier harm reduction treatment and training center in NYC.  She currently runs an ongoing Men’s Sexual Health Group that focuses on men struggling with out of control sexual behavior.  Her interests are in the intersection of psychoanalytic thinking, sex therapy, couples therapy and Integrative Harm Reduction Psychotherapy.


Joshua Grubbs

Joshua Grubbs is an associate professor in the clinical psychology Ph.D. program at Bowling Green State University. Dr. Grubbs’s research program generally focuses on behavioral addictions and the social context of addiction. Additionally, Dr. Grubbs researches the intersection of values and behaviors in determining how people perceive their own actions. Dr. Grubbs has published over 80 peer-reviewed academic articles on topics related to addiction, sexuality, morality, religion, and personality. Dr. Grubbs has also authored over 10 book chapters and has had his worked featured in various media outlets in print, audio, and television formats.

  • Review current conceptualizations of sexual dysregulation

  • Apply a non-pathological conceptualization for sexual behavior problems and out of control sexual behavior.

  • Describe clinician’s ethical responsibility when assessing and treating problematic or out of control sexual behaviors.

  • Recite the OCSB Clinical Pathway for assessment and treatment.

  • Apply six principles of sexual health as an ethical framework for OCSB assessment and treatment.

  • Describe various assessment measures used during an OCSB assessment

  • Examine and demonstrate the information gathering elements of an OCSB assessment process. 

  • Apply and demonstrate the treatment elements of an OCSB assessment.

  • Explain how to create and implement a sexual health plan in OCSB group and individual therapy. 

  • Explain how to evaluate three clinical areas related to OCSB: self-regulation, attachment pattern and sexual / erotic conflicts. 

  • Prepare and demonstrate OCSB group therapy interventions.

  • Demonstrate motivational interviewing techniques to enhance client motivation for sexual health.

  • Explain how to match interventions with the client’s stage of change.

  • Identify provider discomfort with topics related to OCSB treatment and sexual health conversations.

  • Explain how to establish the treatment frame for effective OCSB group therapy

  • Organize structured OCSB group treatment events

  • Demonstrate to providers to facilitate predictable clinical moment in OCSB group therapy

  • Demonstrate here-and-now shame-reduction interventions to resolve erotic conflicts
